Shift:
Monday-Friday
Key Responsibilities:
Develop and implement a comprehensive social services program for residents and families
Provide counseling and address medical-related social needs
Coordinate and facilitate family meetings and ongoing communication between residents, families, and the care team
Advocate for residents’ rights and serve as a liaison among all stakeholders
Manage discharge planning and transitions of care
Deliver exceptional service in a supportive and respectful environment

Qualifications:
Bachelor’s degree or higher (Social Work preferred)
Prior experience providing social services in a healthcare or skilled nursing setting
